First make sure that the "qlini_mode" is NOT set to "Disabled", preferably its set to "exclusive" . you can do that in "/etc/modprobe.d/qla2xxx_scst.conf"

Second Load the kernel modules:
sudo su

modprobe -a scst, scst_tape,

iscsi_scst(?)

qla2xxx_scst must be loaded last?
